The management key is required to revoke or reinstate the instrument, upload the document, and issue retrieval passwords. It cannot be recovered if lost. It is never shown again and never stored in readable form.
Choose how to attach verification to the instrument: the signature & verification page puts the execution signatures on the same page as the barcode — so the verification page cannot be removed without visibly breaking the execution of the document. Use it when the POA is being signed (or re-executed). The rider is a separate attachment page for a POA that has already been signed.

Execution Details
Record how the instrument was executed — these details appear on every status check, so a bank or title company can match the paper in hand against the registration: same parties, same date, same witnesses and notary, same page count. For a POA created with the builder, add them here after signing.
Safeguards & Alerts
A power of attorney terminates at your death. Your emergency contact is the person you trust to report it, so relying parties stop honoring the instrument immediately.

Record the principal, at or near the time of execution, answering the standard questions below on camera. On the recording the principal should state their name, confirm they understand what they executed, confirm they acted free of duress, identify whom they appointed as agent, read out the powers (letters) they initialed, and affirm the document reflects their wishes. The video is stored with the registration as contemporaneous evidence bearing on capacity and voluntariness — it is disclosed only to parties holding a retrieval password, never by a bare status check. It is evidence, not a legal determination of capacity.

Nevada requirement: if the principal resides in a hospital, residential facility for groups, skilled nursing facility, or home for individual residential care when the power of attorney is signed, Nevada requires a certification of competency from a physician, APRN, psychologist, or psychiatrist attached to the instrument (NRS 162A.220). Print the blank certificate below, have it completed at execution, and keep the original with the signed document.
Keep the completed certificate on file with this registration. The stored certificate is disclosed together with the document to parties holding a retrieval password — so a bank or title company sees the competency certification alongside the instrument.
Retrieval Passwords
Issue a one-time password to a specific third party (a bank, a hospital, opposing counsel). They combine it with the verification code on the verification page to view and download the full document — once; the password burns on use. Each password is .

Recovery
Lost your management code?
Your management code is never stored — only a one-way hash of it — so no one, not even POALock, can look it up. If you have lost it, you can request a reset by proving your identity. The request is held for a 7-day notice period (your agent and any watchers are notified), reviewed by POALock, and only then may you set a new management code. Losing the code does not stop you from revoking your power of attorney legally — you can always revoke by signing a written revocation and giving notice to your agent and any institutions relying on it; the reset only restores self-service status changes here.
