Verify a Power of Attorney

If you have been presented with a power of attorney bearing a POALock barcode, enter the verification code printed on the instrument below. You will be told only whether the instrument is currently ACTIVE or has been REVOKED — a status check never discloses the document itself.

Step One

Status Check

Scan the barcode on the instrument, or type the code exactly as it appears (for example, LOCK-XXXX-XXXX-XXXX). Do not rely on any power of attorney whose status is shown as revoked, or whose registry code cannot be found.

A status check is $15 per check, payable before the result is shown. Banks, title companies, and law firms that verify regularly save with a monthly plan — from $99/month; high-volume institutions can call (212) 608-1660 for the institutional program.

⚠️ A power of attorney terminates at the principal's death. This registry reports the status most recently declared; it cannot know of a death that has not been reported. For significant transactions, independently confirm the principal is living.

Death of the Principal

Report that a principal has died

A power of attorney terminates automatically at the principal's death — but the paper doesn't know that, and neither does anyone relying on it. If a principal registered here has died, file a report below: every subsequent status check on their instrument will carry a prominent do-not-rely warning while the report is verified. Reports require your name, relationship, and contact information; knowingly false reports are actionable.

Proof of death (optional). You may attach supporting proof. Nothing you upload is shown to others in full — verifiers who pay to view see only a redacted record: the type of proof and the non-sensitive corroborating details you enter below. The uploaded file itself is encrypted and never disclosed. Do not upload documents containing a Social Security number or cause of death if you can avoid it.

Step Two · With Authorization

Retrieve the Full Document

If the principal has issued you a one-time retrieval password for this instrument, you may view and download the complete executed document — once; the password burns on use, so save what you retrieve. A status check alone never grants access to the document.

Pre-launch preview. The registry currently runs in pilot mode: registrations are stored in the browser in which they were created. At launch, verification will run against the registry's secure server and codes will be checkable from any device.
